Every one of those movements loads the body. The forces generated by walking, lifting, sitting, and standing travel through the joints, the spine, the fascia, and the connective tissue in patterns that are either mechanically efficient or mechanically costly. When they're efficient, the load distributes across the system and no single structure absorbs more than it can handle. When they're not, load accumulates in the structures least equipped to carry it, and those structures eventually respond with pain, inflammation, or injury.

What we actually assess
Standard assessment in physiotherapy and chiropractic identifies what structure is painful or damaged. This is useful and we don't dismiss it. But it answers the wrong question first.

Gait
Gait is the first thing we assess because it's the most repeated loaded movement the body performs. How you walk determines how your body loads, compensates, and adapts across thousands of daily repetitions. Most chronic lower body and spinal pain presentations have a gait component. Most are never assessed from this angle.
We look at how the hips are extending through the push-off phase. We look at what the pelvis is doing across the stride cycle. We look at whether the thoracic spine is rotating as it should, driving arm swing and dissipating rotational force. We look at how the foot is striking and loading. And we look for the asymmetries, restrictions, and compensations that the body has been quietly running for months or years.
Posture under load
Static posture tells us what position the body defaults to at rest. But pain lives in movement, not in static positions. We assess how postural patterns change when the body is loaded, because that's when dysfunctions become mechanically significant.
Fascial tension patterns
The fascia is the connective tissue system that links every muscle, joint, and organ in the body into one continuous web. Restrictions in the fascial system, often from old injuries that healed without full rehabilitation, alter how force travels through that web and create loading patterns that express as pain in locations seemingly unrelated to the original injury. An ankle restriction can produce knee pain. A hip restriction can produce shoulder dysfunction. The connection is real and it's fascial.
Breathing mechanics
Breathing is the most repeated movement of all, occurring approximately twenty thousand times per day. How you breathe directly affects how the thorax loads, how the diaphragm functions, and how the whole trunk stabilises under movement. Dysfunctional breathing patterns are a consistent finding in people with chronic neck, shoulder, and upper back pain, and are almost never assessed outside of respiratory medicine.
What mechanotransduction means for your pain
Mechanotransduction is the process by which mechanical forces are converted into biological signals at the cellular level. It is the mechanism through which movement heals tissue.
When load is applied to connective tissue in the right direction, at the right magnitude, the cells within that tissue respond by remodelling. Collagen fibres align along lines of force. Density and organisation improve. The tissue becomes more capable of handling the loads placed on it.
This is why the right movement, applied correctly, does not aggravate injured tissue. It rehabilitates it. The problem is that most rehabilitation either avoids load entirely, which prevents mechanotransduction from occurring, or applies load through the same dysfunctional pattern that created the injury, which continues to stress tissue in the wrong direction.
